Team captains are encouraged to review the Official Eligibility Rules prior to submitting their team registration.
Registration deadline is May 15, 2004.
Team registration for the ADA National Championship MUST be done via the secure online form. Team event and Single event entry fees may be submitted online with Visa or MasterCard, or by mailing a money order directly to the ADA headquarters.
Team Captains make certain to gather all the following information from each of your team members prior to proceeding to the online registration form:
- ADA Membership number
- First and Last name
- Current mailing address
- Social Security Number
- Singles event participation (Pre-registration only. After team registration and approval, no singles registration is allowed)
- Non-team event format (Neutralizer or Open for ALL their singles, doubles, mixed doubles and mixed triples events)
- Lowest PPDA (the lowest PPDA they should be allowed to compete at to maintain fair competition)
After submitting the team registration, you will be directed to a Team Certification Statement page. To complete your team registration process, this page MUST be printed, signed by all team members and mailed directly to the ADA headquarters. The signed Certification Statement MUST be received no later than May 24, 2004.
